Instructions:  Please read and enter a response for each of the following 23 questions based on your perspective and experience with the organization. The survey has to be completed during one session (it does not allow "Save & Continue).  It should take approximately 15-30 minutes, based on how much detail you choose to provide for the open-ended questions.

Your individual responses will not be shared with anyone. The consultant is the only individual who will have access to the raw data from this survey. After all board members have responded, a summary report will be prepared but will not include any references to identify individual board members.

    For each task or role for questions 1-10, check the box in front of the response that is most appropriate to indicate your current level of participation as a board member.  The response are rated as follows:  10=Always; 7=Most of the time; 5=Sometimes; 3=Rarely and 1=Never.
